Workplace Incident Reporting and Investigation

Overview

This course is designed to introduce participates to the concept and basic knowledge of incident investigation; theory, approach, investigation process, methods and tools, and reporting of incidents including follow-up.

Learning Objectives

The learning objectives for this course are:

1. Understand the 5 W’s of Incident reporting (Who, What, Where, Why, When)
2. Understand the different types of incidents that require investigations
3. Learn how to properly conduct an investigation and create reports

Who should take this course?

• Anyone who wants to increase their knowledge of workplace health and safety. 
• Key staff who are involved in conducting work site investigations and reporting injuries, work refusals, near missed and more.

Course Prerequisite

Required: None

Recommended: Joint Worksite Health and Safety Committee / Representative training

Duration: 4 hours (1/2 day)

Please note: It is expected that all students will actively participate in the discussion and class exercises to ensure that an understanding and learning objectives for the course are achieved. Students MUST have access to a computer with video and audio capabilities. Minimum passing grade for final exam is 80%.
